WHAT TO EXPECT AT YOUR FIRST VISIT
Ashburnham Foot and Ankle Centre kindly asks that you arrive 10–15 minutes early for your first appointment. This allows time to complete any necessary paperwork and ensures you are ready for your scheduled appointment.
To save time during your visit, you may print and complete the intake form below before arriving. A medical history is required, so please bring a list of your current medications with you.
Our foot specialists also recommend bringing two pairs of your regular footwear. Examining wear patterns on your shoes can provide valuable information during the clinical assessment. Certain footwear features may also contribute to, worsen, or help relieve foot-related concerns. If your condition is related to your gait or walking pattern, we recommend wearing or bringing a pair of shorts, if possible, to allow for proper assessment of the feet and legs.
During your visit, our specialist may take photographs of your feet and legs for clinical reference. In some cases, a video of your walking pattern may also be recorded. This allows us to review your gait frame by frame to identify subtle movement patterns or irregularities.

INTAKE FORM

To save time at your first appointment, you may print and complete the intake form below before arriving. If you prefer, the form can also be completed at the clinic—we simply ask that you arrive 10 minutes early.

DIRECT BILLING
Ashburnham Foot and Ankle Centre is able to direct bill most insurance companies. Please note that we can only dual submit claims when the secondary insurance provider is Green Shield. Coverage for chiropody services varies depending on your specific insurance plan, so we recommend contacting your insurance provider directly to confirm the details of your coverage. In some cases, insurance payments are made directly to the clinic, while other plans reimburse the patient directly. If your plan reimburses you, payment will be required at the time of your appointment and your insurance company will automatically reimburse you according to your plan’s coverage.

In order for us to direct bill your insurance provider, a completed Electronic Submission Consent Form is required. You may print and complete this form ahead of time and bring it with you to your appointment, or you can complete the form at the clinic when you arrive.

​PLEASE NOTE: If your insurance provider is Sun Life, please complete the Sun Life consent form provided below. This form must be completed and signed by the primary plan member in order for us to submit claims on your behalf.
