WHAT TO EXPECT AT YOUR FIRST VISIT:
Ashburnham Foot and Ankle Centre would appreciate you arriving 10-15 minutes early for your first appointment. This helps to ensure the relevant paperwork is completed and you are ready for your appointment time. If you would like to save some time you can print the Ashburnham Foot Centre intake form below. A medical history will be required so please bring a list of your current medications with you.

The foot specialists at our foot centre suggest bringing two pairs of your regular footwear. Observing patterns of wear in your footwear can be a valuable tool in the clinical assessment process. In addition features of footwear can sometimes cause, exacerbate or help relieve an issue. If your problem is gait related a pair of shorts will be needed for the assessment of the foot and leg if possible.

Our specialist may take a photo of your feet and legs for reference. Sometimes a video of your walking pattern could also be taken. The video can be broken down frame by frame to identify subtle changes or patterns in your gait.

Parking is available at the rear of the Ashburnham Foot Clinic. Alternatively there is 1 hour parking on Hunter St East or 3 hour parking on the side streets.
